The Advanced Practice Provider is responsible for providing quality care for patients and increasing patient access and satisfaction. Standard tasks to be performed by a NP/PA as specified by the Ohio State Board of Medicine and UH entity specific Medical Bylaws.

The CAN Kidney program serves patients with advanced chronic kidney disease (CKD), addressing gaps in care and navigating the difficult transition from advanced CKD to dialysis and transplant. Additionally assisting with current and new research study opportunities.

We are seeking a compassionate and experienced Part-Time Advanced Practice Provider to join our team and provide specialized care for patients with advanced chronic kidney disease (CKD). In this role, you will be responsible for educating patients on renal replacement therapies (including dialysis modalities and kidney transplant options), facilitating access to vascular surgery and transplant surgery referrals, and coordinating care with nephrology teams to ensure optimal dialysis starts and preemptive transplant options. This position is ideal for an NP with expertise in nephrology and a passion for improving the lives of patients with advanced kidney disease.

Qualifications

Education Qualifications

· (MSN) Master's Degree in Nursing, or

· Master's Degree from an accredited PA educational program

Experience Qualifications

· Minimum of 1 year of experience in nephrology, renal care, or related field, preferably working with advanced CKD, dialysis, and kidney transplantation.

Skills and Abilities

• Strong knowledge of CKD progression, dialysis modalities, kidney transplantation, and nephrology care.

• Excellent communication skills to educate patients, families, and the healthcare team.

• Ability to collaborate effectively with nephrologists, dialysis centers, and transplant team 
\n
• Proficiency in Electronic Medical Records (EMR) systems, Microsoft Excel, Word, Outlook.

Licenses and Certifications

\n
• Current Registered Nurse (RN) in the State of Ohio, and

\n
• Nurse Practitioner (NP) in the State of Ohio, or

\n
• Current Physician Assistant (PA) in the State of Ohio

\n
• Basic Life Support (BLS) Required and

\n
• Advanced Cardiac Life Support (ACLS) Preferred
