Summary
A community health worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
A community health worker also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
There are certification levels of 1 - 4 in North Carolina. The person can meet any certification level (1 - 4). The person will work 40 hours a week and provide care coordination, site visits, linkage to outside agencies for referral, coordination with probation/parole, reentry, and follow-up care post release.
Responsibilities
- Educate community members about mental health and substance abuse resources and services available.
- Build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
- Assist clients in navigating healthcare systems and accessing necessary services.
- Collect data on community needs and program effectiveness to inform service
- Work closely with primary care and other physical health providers
- Facilitate public speaking events to raise awareness about mental health and substance abuse issues.
- Connect individuals with essential social services such as housing, employment, food assistance, and other resources to support their successful reentry.
Requirements
- Must have at least a High School diploma or equivalent with at least 4 years of relevant experience in public health or a bachelors degree in a related field.
- Experience in public health or mental health and substance abuse is required.
- Strong communication skills, both verbal and written, for effective education and advocacy.
- Familiarity with medical terminology and healthcare systems is beneficial.
- Experience in addiction counseling or working with individuals with disabilities is a plus.
- Ability to collect data and analyze information to improve service delivery.
