Posterity Group LLC is a veteran owned, service-disabled small business, headquartered in Rockville, MD, that specializes in federal government contracting and staffing. We are tasked with staffing The 92nd Medical Group at Fairchild AFB near Spokane with Case Manager Registered Nurses. The Registered Nurses are expected to perform the following:

  • Proactively identifies and evaluates patients and families for case management from a variety of sources such as discharge/disposition planning, referrals, the Medical Evaluation Board process, the healthcare system, employers and facility staff.
  • Conducts systematic, on-going, thorough collection of patient’s physical, emotional, psychological, social and medical status. This information is obtained via direct patient contact and other relevant sources such as professional and non- professional caregivers, medical records, and family/caregiver interviews.
  • Develops an appropriate patient-specific plan of care to include short- and long term goals, objectives and actions.
  • Coordinates, collaborates, and obtains approval of the plan among the patient, family/caregiver, primary care provider and other members of the healthcare team and documents in the EHR.
  • Guides the patient and family/care giver through the healthcare system, maximizing use of resources.
  • Coordinates and executes the plan of care, optimizing access to appropriate services.
  • Ensures necessary referrals are ordered by the appropriate discipline and coordinated.
  • Serves as an advocate for (and ensures education is provided to) the patient and family/caregiver, as required, and promotes adherence to treatment plans for improved healthcare outcomes.
  • Ensures coordination of care delivery processes, to include alternate healthcare settings and the home environment, for the purposes of enhancing the patient's health and wellness, safety, productivity, quality of life, and for providing the most beneficial, cost-effective health care.
  • Develops, utilizes, and maintains a variety of military and community resources to optimize access to services and medical care, and ensures timely and appropriate provision of services.
  • Documents and updates treatment plans in electronic health record (EHR) as needed and maintains documentation and data collection in accordance with existing DOD, AF, DHA, MTF, and other agency guidelines.
  • Monitoring and evaluation may include patient’s adherence and response to the treatment plan; timeliness of patient and family/caregiver contact and follow-up; identification of variances, patterns or trends from established practice guidelines and/or standards; established outcome measurements; results of interventions, treatment delivery and timeliness of care; and utilization of resources.
  • Monitors and evaluates the facility’s case management program per DOD, AF, DHA, and local policies and guidelines.
  • Coordinates and participates in interdisciplinary team meetings, designated facility meetings, and Care Coordination meetings.
  • Shares knowledge and experiences gained from own clinical nursing practice and education relevant to nursing and case management.
  • Participates and adheres to orientation, education and training of staff and facility.
  • May serve on committees, work groups, and task forces at the MTF.
  • Shall maintain a level of productivity and quality consistent with: 1) complexity of the assignment; 2) facility policies and guidelines; 3) established principles, 4) ethics and standards of practice of professional nursing; 5) the Case Management Society of America; 6) American Accreditation Healthcare Commission; Health Services Inspection; and other applicable DOD and AF specific guidance and policies.
  • Shall also comply with the Equal Employment Opportunity (EEO) Program, infection control and safety policies and procedures.
  • Completes medical record documentation and coding and designated tracking logs and data reporting as required by local MTF/AF/DOD/DHA instructions, policies and guidance.
  • Completes all required electronic medical record training, MTF-specific orientation and training programs, and AF/DOD/DHA mandated Case Management training.
  • Knowledge and ability to effectively apply the following core case management functions: a) Assessment: identification of patients for case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care; c) Facilitation: care coordination and communication among all involved parties; d) Advocacy: support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.
  • Provides comprehensive healthcare services, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
  • Assess patients, educate them on health maintenance, administer medications, and coordinate care with other healthcare professionals when required by facility.
  • May see the Armed Forces members and family members in the Primary Care, Flight Medicine, and Mental Health Clinic, as necessary. The Chief of Nursing may assign contract personnel as required to sections/departments to provide support commensurate with the individual’s experience.
Requirements:
  • Degree/Education: Proof of graduation from an accredited educational institution with a Bachelor of Science in Nursing (BSN) degree or higher.
  • Valid BLS
  • Shall possess active, valid, current and unrestricted license to practice as a Registered Nurse in any of the 50 U.S. States or Territories with no limitations, stipulations or pending adverse actions.
  • Shall have a minimum of two-years’ experience, within the last four years, in professional nursing or Case Management for adults, children, families, seniors, or groups.
  • Case Managers assigned to a specialty care area shall have two years minimum experience in that specialty.
  • Contract personnel shall demonstrate expert competency in performance of core activities of Case Management and Discharge/Disposition Planning, i.e., patient advocacy, assessment, planning, implementation, coordination, monitoring, and evaluation.
  • Expertise in resolving complicated healthcare, social, interpersonal, and financial patient situations is essential.
  • Shall have experience in program planning, implementation, and evaluation, and in conducting individual, family, group, and community assessments.
  • Personnel shall also have the ability to communicate effectively both orally and in writing. o Knowledge of medical privacy and confidentiality, including but not limited to HIPAA, accreditation standards of Accreditation Association for Ambulatory Health Care and The Joint Commission. o Must be skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous. o Must demonstrate ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal, and financial patient situations.
  • Must possess organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way.
  • Familiarity with DOD, Federal, State, non-profit healthcare systems and organizations, to include DOD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is desired.
